Full-time (3:00pm - 11:00pm, and 2 weekends per month). The Team Leader serves as a manager and member of the Acute Care Services clinical team, which includes the Care Center, HospiceCare Plus Program, and On-call services. The Team Leader provides clinical support and coordination for the care provided to patients/families served by designated interdisciplinary team members and supervision of staff assigned to the teams. Leads and coordinates the provision of services in a manner that promotes an interdisciplinary team approach to care that respects patients/families, meets Medicare/Medicaid conditions of participation and reflects the mission and vision of the agency. Current Colorado RN license required, BSN preferred. 2-3 years of experience in a setting that provides end-of-life care required.
Full-time, M-F. Also rotates one week night of On-Call every six weeks. This position is responsible for providing follow-up grief services to hospice families after a hospice death, facilitating grief groups, and taking community calls for grief support. Qualified candidate must possess a minimum of a master's degree in human services related field or counseling, LPC or LCSW preferred; have experience working with grief, including facilitating grief groups; and be at ease with public speaking and presenting in front of large groups.
Full-time. This position is responsible for recruitment activities for all positions in the organization, assisting with new employee orientation, coordinating and implementing retention programs, conducting exit interviews and other HR generalist responsibilities. Requires excellent interpersonal skills. Proficiency in Microsoft Office required. Bachelor's Degree in HR or a related field, HR certification or the equivalent experience; and previous experience with HR information data base (HRIS) and in a health care environment preferred.

Full-time, M-F, 8 am - 5 pm. The Medical Records Associate is responsible for assisting the Medical Records Coordinator in management of electronic and paper Medical Records in compliance with the Medicare Conditions of Participation. Excellent organizational skills, ability to multitask and meet deadlines, ability to work/coordinate with many IDT members and clinical managers. Excellent communication skills. Excellent knowledge of and proficiency with Microsoft Windows, Microsoft Word, and Excel. Observant and respectful of confidentiality principles. Proficient with medical terminology. Previous related experience in a medical environment required.
Full-time positions for HospiceCare Plus Program, providing one-on-one care to patients residing in homes and nursing homes. Evening and overnight hours (9:00pm - 9:00am) weekdays and weekends. Current Colorado CNA license required.

Provides direct patient care, collaborates with the other members of the care team and with other health care providers in various care settings. 1-2 years of experience in a setting that delivers end-of-life care required. Current Colorado LPN license required.
Part-time (.5) position available for candidate with an MSW and a minimum of one-year experience in medical setting that delivers end-of-life care, as a relief to Team Social Workers in all geographical areas, and may also cover Care Center Social Worker role, as needed. Collaborates with the other members of the clinical care teams, in Home Based and Acute Care Services, and with other health care providers in various care settings.
Full-time, 8:00 am - 6:00 pm, four days per week. A minimum of one year previous medical office experience, strong computer and administrative skills, and excellent customer relations skills required. Previous hospice experience preferred.
